Současný pohled na diagnostiku a léčbu systémové sklerodermie a Sjögrenova syndromu

Systémová sklerodermie (SSc) je generalizované onemocnění pojivové tkáně nejasného původu, které se projevuje fibrózou a sklerotizací kůže, stěny cév a parenchymu viscerálních orgánů. Postihuje častěji ženy v mladším a středním věku. Postižení pohybového ústrojí signalizují myalgie, artralgie, artritidy a třecí šelesty šlachových pouzder. Postižení trávicího traktu je časté a objevuje se ve všech jeho částech. Plicní postižení mívá formu intersticiálního procesu nebo plicní hypertenze. Postižení srdce se manifestuje dušností, poruchami rytmu a převodními bloky. Nejsou k dispozici žádné léky s účinkem na průběh SSc a tedy léčba je převážně symptomatická. Sjögrenův syndrom (SS) je charakterizován postižením žláz s vnější sekrecí, kdy dochází k lymfoplazmocytární infiltraci jejich parenchymu s následnou poruchou funkce. Onemocnění může být primární nebo provázet jiné autoimunitní choroby. Postihuje zejména ženy ve středním věku. Bývají postiženy žlázy slzné, slinné, trávicího traktu, dýchacích cest, kůže a vaginální sliznice. Mohou se i objevit extraglandulární projevy jako artritidy, vaskulitida, Raynaudův fenomén, renální abnormality a neuropatie, vzácný je přechod v maligní lymfom. Léčba glandulární nemoci je symptomatická, při mimožlázových manifestacích je indikována imunosupresivní terapie.
