The article provides a summarized overview of the new guidelines for the diagnosis and management of dyslipidaemias that were published in 2019 as a joint effort of the European Society of Cardiology and the European Atherosclerosis Society, and were commented on in a statement of the Czech Society for Atherosclerosis. The new guidelines lower the target LDL cholesterol levels in all risk categories, introduce new SCORE charts for risk assessment, and emphasize the use of calcium score, ultrasound scan of carotid or femoral arteries, and modifying factors (social deprivation, psychiatric comorbidities, left ventricular hypertrophy, atrial fibrillation…) in those with a moderate risk. In younger people, the use of a relative risk chart or estimation of vascular age are recommended.
