Whom and how to treat for pulmonary hypertension

doc.MUDr.Pavel Jansa, Ph.D.

Med. praxi. 2013;10(10):318-320

Pulmonary hypertension encompasses conditions in which mean pulmonary artery pressure exceeds 25 mm Hg. It most commonly occurs in heart and lung diseases. More rarely, pulmonary hypertension is a result of primary involvement of the pulmonary vessels (particularly pulmonary arterial hypertension with involvement of the pulmonary arterioles and chronic thromboembolic pulmonary hypertension associated with inadequate reperfusion following acute pulmonary embolism). It is the treatment of the underlying heart disease that is crucial in the management of pulmonary hypertension in heart diseases. Specific vasodilation therapy is not indicated in...

Practical aspects of care of a patient with chronic venous disease

MUDr.Jana Vojtíšková

Med. praxi. 2013;10(10):321-324

Given the aging of the population, increasingly more attention is being paid to the quality of life. The figures showing how the world’s population is affected by venous disease are very high, and the Czech Republic is not an exception. The issue of chronic venous disease was neglected in general practice for years. It was also due to the fact that particularly elderly patients tend to trivialize venous complaints and rather attribute them to overexertion or age, so they do not even report them spontaneously. They usually seek consultation at the point when the symptoms of the disease become more troubling or frighten them (e.g., lower...

MUDr.Jan Šťovíček

Med. praxi. 2013;10(10):325-328

Functional dyspeptic disorders are a group of conditions in which the patient’s symptoms are not determined by organic disease, but by impaired organ function. The nomenclature and diagnostic criteria for functional dyspeptic disorders are defined in the Rome III criteria from the year 2006. One of the most common forms of the disease is functional dyspepsia characterized by epigastric pain and/or postprandial fullness and/or early satiation. Gastroduodenoscopy is the fundamental diagnostic method to rule out organic disease; other methods of examination are complementary. According to the type of predominant symptoms, the treatment of...

Year-round care of atopic and very dry skin

MUDr.Lucie Růžičková Jarešová

Med. praxi. 2013;10(10):344-346

Seasons of the year are characterized by a repeated, regular change of the weather. Air humidity and temperature are one of the basic factors that change during the year. Change of the weather is perceived individually by each person. The development of dry skin depends on the action of extrinsic and intrinsic factors. When the integrity of the skin surface is impaired or the skin is not sufficiently hydrated, its barrier function is limited. The skin becomes dehydrated and rough, losing its glossy appearance.

Calcium - bone and cardiovascular effects

MUDr.Mária Rašková, Ph.D.

Med. praxi. 2013;10(10):340-343

Calcium and vitamin D play an important role in primary prevention of osteoporosis and secondary prevention of fractures (1). In adults, calcium is important to maintain the bone mass. Lack of calcium leads to secondary hyperparathyroidism associated with increased bone loss. Prolonged severe calcium and vitamin D deficiency can lead to the development of osteomalacia. Recent studies have suggested an increased cardiovascular risk in long-term supplementation with higher doses of calcium, especially for those with sufficient dietary calcium intake (total intake of 1 400 mg of elemental calcium daily) (2). It is advisable to pay attention to...

In-home visiting service as part of responsibilities of nurse working in general practitioners office

Bc.Michaela Šuplerová, Mgr.Alena Machová2

Med. praxi. 2013;10(10):353-354

A significant benefit of the nurse’s visits in patient’s home is mainly the regard for patient’s private space. The patients recover better in home conditions than in hospitals or other social facilities. They are provided with individual care according to their specific needs. The economic factor is also important. The nurses employed in the GP’s surgery for adults who regularly visit their patients are familiar with the entire health documentation of the patients and their families, as well as their social conditions. This is why the care is more effective. Another important factor is a faster sequence of nursing and...
